Check List and Forms

Please complete and submit the following forms, which are available online at http://www.mtholyoke.edu/acad/firstyear/11728.shtml

  • Summer Experiences Questionnaire  (Before you can register: login to ISIS, select the "For Students" menu, select "Inform Your Advisor" and, in the "Summer Experiences" section, click the box under "Update" at far right.)
  • New Student's Initial Infor for Advisor Questionnaire (In November: login to ISIS, select the "For Students"menu, select "Inform Your Advisor" and then click on the link under "New Student's Initial Info for Advisor." The information you include will be critical for your work with your faculty advisor, and will be shared only with those individuals who can further assist in advising you.)
  • Language Questionnaires & Exams (Due by December 1st) All students who wish to study French, German, Spanish, Latin, Italian, or Russian at Mount Holyoke must complete a Language Proficiency Questionnaire. This information is needed to place you in the course that is most appropriate for you. If you wish to study French, German, Italian, Latin, Spanish, or Russian, be sure to take the appropriate placement exam.
  • Mathematics Assessment Exam - if applicable (Due by December 1st) The Mathematics Assessment Exam is for students planning to begin the study of calculus.
  • Accommodations For Students With Disabilities- if applicable (Due by December 12th) If you will require accommodation due to documented disability, please complete necessary forms.
  • ISIS - registration for spring entrants starts 3pm November 12 and ends Friday, November 20.
  • Preliminary Transfer Credit Evaluation Request- if applicable, Check here in December for directions and the specific due date in December.

Transfer Credits-Please send an official transcript (not a grade report) for all the courses you hope to receive credit to Mount Holyoke's Registrar as soon as the term is over.
