Checklist
For Students Approved for Academic Leave of Absence
Deadlines: May 15 for fall/full year, December 1 for spring.
- Return your library books.
- If you want to continue on the Mount Holyoke student health insurance plan while you are on academic leave, submit a written request to the Office of Student Financial Services.
- Review your plan of study with your academic advisor, particularly in regard to any courses you wish to use to fulfill requirements.
- Leave a forwarding address with Mail Services.
- Forward your MHC e-mail if you will not be using your account here while you are away. Important information about pre-registration and housing will be sent to your MHC e-mail account, so be sure that you continue to access those messages and keep that account clear of any quota restrictions.
- If you have been awarded a Laurel Fellowship and/or expect to use any federal, state, or other outside aid, or parent loan or payment plans toward your program abroad, see additional forms and instructions.
- Check with Student Financial Services before you leave campus to ensure that your file there is complete (if applicable).
