Medical Forms

Mount Holyoke athletes must complete two forms each year in order to participate in a varsity athletics program:

1. the Emergency Information form, which provides health insurance and emergency contact information, and
2. a medical history form, which provides details about medical conditions and overall health. All new athletes must complete the form for New Athletes, while returning varsity athletes must complete the  form for Returning Athletes.
