Medical Forms

Mount Holyoke athletes must complete two forms each year in order to participate in a varsity athletics program:

1. The Emergency Information form, which provides health insurance and emergency contact information, and
2. A medical history form, which provides details about medical conditions and overall health. All new athletes must complete the two sided athletics pre-participation form included with the the physical required for admission to Mount Holyoke College.  This form is sent to admitted students late spring/early summer by the Health Center.  Returning varsity athletes must complete and submit the online form for Returning Athletes.
