Application Instructions
1. Download or pickup a membership packet from the Kendall Membership Website or the Kendall Hall Information Desk. Membership packets consist of a membership agreement, a copy of the rules for using Kendall and a waiver of liability. Each person enrolling will need to complete his/her own packet.
2. Return the completed packet and payment to the Kendall Hall Information Desk. Enrollments will be processed as quickly as possible.
3. If you are an employee of the College, a copy of your membership agreement will be sent to you via campus mail. You will need this in order to obtain reimbursement from your health insurance provider.
4. Mount Holyoke Employees can then download or pickup a Blue Cross/Blue Shield Fitness Benefit Form (English | Spanish) from the Kendall Membership Website or the Blue Cross/Blue Shield Website. Forms are also available at the Kendall Hall Information Desk and the Office of Human Resources in Skinner Hall.
5. Bring or send both the copy of the Kendall Membership agreement and the Fitness Benefit form to Human Resources for batch processing by March 15, 2010. Special arrangements have been made for this year only. Reimbursement requests are typically mailed directly to Blue Cross/Blue Shield and are processed after the subscriber has been enrolled with their fitness center for four months. For this year only, Blue Cross/Blue Shield will waive the four month period and process reimbursements for enrollment at Kendall through special processing via the Mount Holyoke Human Resources Department.
Any reimbursement requests received in human resources by March 15, 2010 will be eligible for this processing. Requests received after March 15 will be processed using the normal Blue Cross/Blue Shield Channels.
