Application Form and Instructions

For Students

  • Access the online UAF Application Form by logging into ISIS and selecting "2010 Universal Application Form for Summer Funding" from the list of options.

    Applicants must complete and submit the following two forms by hard copy to the CDC by February 26, 2010, by 5 p.m. in order to finalize the UAF application:

    1.  UAF Confirmation Form (to be completed by ALL applicants)
    2.  UAF Off-Campus Travel Agreement Form (to be completed by students proposing off-campus projects, including projects that are abroad)  This form will become available by November 25, 2009.
