To apply, access the online UAF Application Form by logging into ISIS and selecting "2012 Universal Application Form for Summer Funding" from the list of options.

Required Forms
All applicants must complete and submit the following two forms by hard copy to the CDC by February 24, 2012, by 5 p.m. in order to finalize the UAF application:

• UAF Confirmation Form
• UAF Sponsored Travel Waiver Form

Forms can be sent by mail to Mount Holyoke College, Career Development Center, 50 College Street, South Hadley, MA 01007 or by fax to: 413-538-2081

Please Note: Students sending these forms from overseas should call 413-538-2080 or email CDC to confirm the receipt of their documents.
