Required Forms

Please note that these forms are available to fall entrants at the end of May and to spring entrants at the end of October (and not before).

New Student's Initial Information for Advisor Questionnaire

This questionnaire is mandatory for all students and must be submitted by June 30 (fall entrants) or December 1 (spring entrants).  To complete the questionnaire, login to ISIS, select the "For Students" menu, select "Inform Your Advisor" and then click on the link under "New Student's Initial Info for Advisor."  The information you include will be critical for your work with your faculty advisor, and will be shared only with those individuals who can further assist in advising you.

Registration with Disability Services

If applicable, due December 12, register with Disability Services (through ISIS) to request accommodations under the Americans with Disabilities Act.    

Summer Experiences

Spring entrants only (due November 10):  Login to ISIS, select the "For Students" menu, select "Inform Your Advisor" and, in the "Summer Experiences" section, click the box under "Update" at far right.  For a visual guide of these steps, please see the registrar's instructions.

Transfer Estimate Request

Spring entrants only (complete between December 12 and January 6): If you have taken any courses at a college or university that you wish to transfer to Mount Holyoke, please complete this form.  Login to ISIS, select the "For Students" menu, and click on "Transfer Estimate Request(s)" under "Academic Planning".  For a visual guide of these steps, please see the registrar's instructions.  To receive credit, please be sure to have an official transcript (not a grade report) sent by the institution where you took the courses to the Office of the Registrar.
