1. Online Application

Brief autobiographical statement and 2-3 page graded academic paper or optional essay that describes an intellectual pursuit or experience that has had special value for you.
- email, fax, or send writing samples to Frances Perkins Office
- essays do not have to accompany the application form but they should be sent by the appropriate deadline
4. Personal Résumé
- Your résumé should include employment history, community service activities, and any scholastic distinctions or honors you have earned email, fax, or send the résumé to The Frances Perkins Office.
5. Secondary School Transcript and/or GED
- Send to Frances Perkins Office
6. College Transcripts
- Send to Frances Perkins Office
7. Academic Reference Form (PDF)
- two are required if you are enrolled in college classes
- print these forms for instructors or academic counselors and instruct them to mail or fax to Frances Perkins Office
8. Academic Progress Report (PDF)
- if you are currently enrolled in courses that will not be completed before the application deadline, ask instructors to help you complete the progress report
- send to Frances Perkins Office
9. General Reference Form (PDF)
- two are required if you are not enrolled in college; optional if you are
- submitting academic references
- print these forms for your employers or references and instruct them to mail or fax to Frances Perkins Office
10. Financial Assistance Forms: Frances Perkins Financial Aid Application (PDF)
- send to Student Financial Services
- spring enrollment: submit by December 1
- fall enrollment: submit by March 1
Free Application for Federal Student Aid (FAFSA)
- U.S. citizens/permanent residents only: send to Student Financial Services

11. Personal Interview
- Mount Holyoke strongly recommends an interview, either on campus or with an alumnae representative in your area
- all on campus interviews can be scheduled up until the application deadline
- to schedule, contact the Frances Perkins Office
- to schedule an off campus interview, contact the Frances Perkins Office for the alumnae representative in your area.
- please schedule alumnae interviews no later than two weeks before the application deadline
