OPT Information Form

If you will be on OPT after completion of your studies, please complete the form below.  Failure to notify the McCulloch Center for Global Initiatives of your mailing address and employer information will result in your falling out of legal immigration status.  Failure to provide employer information will also result in a termination in the SEVIS system 90 days after the start date on your EAD card.
