formdb Wizard Step 2: Prerequisites

Before continuing with this wizard, you must complete the following steps:
1.Create a web page containing your HTML form. Be sure to give each field its own, unique name. (The one exception to this is a set of radio buttons, which will all have the same name but different values.)
2.Save your web page to the web server. It is usually best to place this file into a folder by itself.
3.Complete the request form, and wait until the Webmaster emails back, saying the form directory has been created for you.
4.With the web page open in your editor of choice, click on the Continue button below.
Until you have completed the steps above, you will not be able to answer many of the questions on the following pages.
