Equipment Reservation Form

Date equipment needed: Reservation should be made at least one week in advance.

Equipment must be requested at least one week in advance.
Quantities are limited and most equipment is in high demand.
Equipment requested with less than 7 days advance notice cannot be processed and will receive an automated reply.
Call ext. 3162 or ext. 2073 if you have questions.

Check all that apply:

		Digital Picture Camera Reservations are required.
		Laptop (PC) Laptop Reservations are for Faculty and Staff only. Wireless laptops for student use are available at the Circulation Desk for 3 hours. Use is limited to the LITS complex only and these laptops cannot be reserved.
		Laptop (MAC) Laptop Reservations are for Faculty and Staff only. Wireless laptops for student use are available at the Circulation Desk for 3 hours. Use is limited to the LITS complex only and these laptops cannot be reserved.
		DVD Player
		VCR
		Computer/Video Projector If you have never used a computer/video projector before, please make sure to set up an appointment with Media Resources for a quick lesson before the date the projector is needed.
		Extension Cord
		Portable Screen
		Slide Projector (for 35mm slides, includes carousel)
		Transparency Projector (old style)
		Portable Sound System
		Compact Disc Player
		Microphone Specify mic type (handheld, shotgun, or lavalier) and input type (mini, xlr or 1/4 inch) in the Special Instructions below.
		Microphone Stand Specify table-top, floor or extendable tri-pod in the Special Instructions below.
		Digital Video Camera* All video cameras record to MiniDV tapes.
		Video Tripod
		Digital Audio Recorder* Requires training prior to use. Please suggest times for appointment.
		Audio Cassette Recorder/Player Please let us know if you need any special features.
		Micro Cassette Recorder*
		Headphones
		Speaker Phone
		Megaphone Requires 8 D batteries (not supplied).
