Summer 2008 Universal Application Form
This page provides access to the online application form for summer opportunities funded by the College. For more information on any of these programs, please return to the main site.

Notes:
- The deadline for applications will be 5:00 p.m. on March 3, 2008. Your entire application must be completed and uploaded by that time. There will be NO exceptions.
- You will receive an email acknowledgment of receipt of your completed application within minutes of clicking "submit" on the last page of the on-line application. Please SAVE that email ! If you do not receive this acknowledgment, your application has not been received -- please contact Katerina King if there are problems.
- The email acknowledgment will include instructions for the hard copy forms that must be turned into the CDC, c/o the Summer Opportunities Committee, by March 3. These include the Summer Fellowship Confirmation Form (to be completed by ALL applicants) and the Off-Campus Travel Agreement and Waiver Form (to be completed by all those proposing off-campus projects).
- You, the student, must contact the faculty or staff member who will write a letter of recommendation for you, at least two weeks before the deadline. [If it's less than two weeks before the deadline and you still haven't requested your recommendation, speak to the recommender anyway. He/she may be willing to provide you with one on shorter notice.] You must provide him or her with this url for the online faculty recommendation. Before doing so, you must log onto the application form (in ISIS), fill in the MHC user name of that faculty member in the appropriate space, and "submit" that first page of the application. You will still be able to return to that page and continue to edit it until you submit the entire application (by pressing "submit" on the second page of the application, after the narratives). The faculty member will be able to see anything you have entered in your application, even if it's not final, as you continue to update it and after it has been submitted. If you are seeking research funding, your recommender should be in a position to evaluate your preparation and ability to carry out the proposed research. If you will be carrying out your research in a foreign country, your recommender should discuss the nature of the support and supervision you will have there.
- Please contact your recommender(s) and share drafts of your application narratives with them BEFORE you submit your application. If your recommender is not an MHC faculty or staff member, the letter can be emailed directly to Darby Dyar (mdyar@mtholyoke.edu) for insertion into your application.
- If you are NOT a Mount Holyoke student, but are applying to the HHMI program, please download the special HHMI form for Five College students here (pdf or MS Word format).
- Should your situation change after you have submitted this application (for example, you receive funding from another source), please notify the organization from which you received funding immediately. Please do not submit your application until AFTER you have contacted your recommender(s) and shared drafts of your application narratives with them.
- Applications from students who have not been previously funded by the College will be given preference over those from students who have already received
funding.
Faculty members wishing to submit supporting recommendations using the on-line form may access it by clicking here. Faculty members please note: the student must enter your MHC user name as part of her application, in order for you to submit her recommendation. If the student's name does not appear when you log into ISIS, then you should contact the student and ask her to be sure to enter your name as part of the first page of the application form, and save it by pressing "submit" at the end of the first page.
