Health Travel Forms

If you are traveling with a study abroad program, you may be required to have a current physical examination, and completion of a health history and physical form. If you have been issued such a form, please complete any portions that are meant to be filled out by you, and bring the form to your appointment. Or, you may submit the online health center travel form 48 hours prior to your health center appointment.
