All offices or departments sending/transferring records to the College Archives and Special Collections must submit this form before sending records to the Archives. Any records transferred to the Archives and Special Collections without submission of this form will be returned to the office of origin.

Please note that this form cannot be sent unless all of the fields have been filled in.
