Event Registration Form
This form, formerly known as "the pink form", is required for all student sponsored events. (Other Campus Departments may also
use it if it helps you to organize your planning process and order the necessary campus services, but you should direct your communications
to the Event Reservations Office c/o Event Services.)
The form is an editable PDF file that can be saved and data fields can be modified
multiple times. To download any of these files, you must have Adobe Reader 6.0 or higher. Please note that lower versions of Adobe will not let you save your work!
It is free to download, just click on the icon to select the correct version
for your computer. Please be sure to "Save as..." the PDF when saving changes!
FORMS ARE DUE 14 DAYS IN ADVANCE OF YOUR EVENT!
(events not properly registered are subject to cancellation and fees)
BOTH Part 1 and Part 2 must be completed!

You might also want to email copies to other event planners on your team so

Part 2: Signature Page
The signature page is part of the Event Registration Form BUT this part must
be PRINTED, COMPLETED, AND a paper copy DELIVERED to the
Office of Student Programs no later than fourteen (14) days before your event.
