Help Create a Web-Based Form

Step 1. Request Assistance

Step 2: Create the Form Questions

  1. Create your form in a Word document.
    Don't worry about adding fields (where users will enter their answers) or tables.
  2. Indicate required fields.
    Include * after all questions that users will be required to answer. If they don't answer a required question/field, an error message will indicate which questions are unanswered. They will not be able to submit the form until all the required questions are answered.
  3. Finalize.
    Have all vested parties sign-off on the form. The less changes we have to make after creating your form, the sooner we can get the form to you.
  4. Recipient names.
    Determine who should receive the form; more than one recipent is allowed.
