Who should use this form?
Students who are already registered with AccessAbility Services should complete this form each semester to renew their academic accommodations.
Accommodation letters for approved accommodations will be available for pickup at AccessAbility Services. Our office is located on the lower level of Wilder Hall.
Along with your accommodation letters, you will receive receipts that your professors should sign and return to our office. Please schedule time to meet with your professors individually and discuss the logistics of your specific accommodations. Please note, letters must be given to professors at least one week before you intend to utilize any accommodation.
Accommodation letters take two weeks to process, please plan accordingly.

- Spring semester: December 1st
