Guidance for Completing the Learning Agreement - This is a guide for Community Fellows to complete their Learning Agreement, required before students may begin as a CBL Program staff member. Learning Agreements are completed by Fellows in consultation with their community site supervisor.
Information for CBL Community Fellow Supervisors - CBL Community Fellows should share this document with their community site supervisor. It contains valuable information for working with college students and specific information helpful to working with Mount Holyoke College and the CBL Program. Students discussing potential CBL Fellowship positions with community partners may also share this document with them to introduce them to this program.

Orientation Manual for CBL Student Staff - A resource guide for all CBL staff, containing a wide-variety of information, policies, guidelines, and essential forms governing CBL employment and practices.
Student Reimbursement Form - Students requesting reimbursement for items they purchased on behalf of the CBL Program must obtain approval from the CBL Director of Community Engagement. They must then complete this form and submit it to the CBL Program Senior Administrative Assistant Diana Fiori in Dwight Hall 217B along with all original receipts as soon as possible in order to be reimbursed.
Student Request for Cash/Credit Form - Students requesting cash or credit for purchases on behalf of the CBL Program must obtain approval from the CBL Director of Community Engagement and submit this form to Sr. Admin. Assist. Diana Fiori at Dwight Hall 217B FIVE days PRIOR to when the funds are needed. A nominal amount of petty cash is kept on hand for such purchases, therefore these types of requests MUST BE made well in advance so that funds are readily available for student purchases.
Fleet Vehicle Reservation Form – All CBL and CAUSE fleet vehicle requests must be made by completing the CBL Fleet Vehicle Request Form and returning it via email to Courtney Brunson at email@example.com with a copy to Angelica Castro at firstname.lastname@example.org in order to be considered. Once completed, the request form will be submitted to Fleet Services. If a driver is required, please state that in the line where it asks for the driver's information. Please note: drivers and vehicles are NOT available for individuals for regular travel to and from sites. If you already have a reservation made and for any reason will not be able to attend, please let Courtney Brunson or Angelica Castro know as soon as possible. If you cancel a reservation, that may allow for other students to use the cars for their projects. Failure to cancel reservations consistently in advance may result in the loss of the privilege to request cars.
Campus Visit Liability Forms for Minors - Fellows and others bringing K-12 students to Mount Holyoke for campus visits are required by the college to have visiting students parents or guardians sign a liability waiver in addition to whatever permission slip the sending school or community organization requires. This form is available in English and in Spanish/Español. For more information, please contact Constance Fontanet at email@example.com.
Photo/Video Permission Waiver Form for Visiting K-12 Students- Fellows and others bringing K-12 students to Mount Holyoke for campus visits or organizing events in the community at which you or CBL staff are taking photos and/or video, need to have the K-12 students' parents or guardians authorize permission to take and use such images. Please check with your community partner - many schools and youth programs already gather such permissions and can advise you. Here is a form, however, in case they have not done so or you are unsure.
CBL Administrative Fellows to assist CBL Staff - All CBL Program Student Staff (Fellows, Mentors, Administrative Fellows) should be aware that there are students employed by the CBL Program able to assist other fellows with photography, communications, transportation, campus visits programming, and other needs. Photography - Please invite either Courtney Brunson (firstname.lastname@example.org), Constance Fontanet (email@example.com), or Rosa Cartagena (firstname.lastname@example.org) to your events on campus and arrange for her to accompany you to off-campus events to take pictures. Please ensure there are proper permissions among program participants (especially youth), and help direct her to avoid taking pictures that identify non-permitted program participants.
News/Blog Posts - Rosa Cartagena can help you develop a blog post to share on the CBL Blog about your project or special event associated with your fellowship.
Transportation - Courtney Brunson at email@example.com. Courtney is our transportation manager. She works with Fleet Services, Five College partners and Assistant Director of Community Engagement Angelica Castro to develop responsive transportation solutions for CBL Program students and projects.
