Nomination Form

  • Nominations are due by noon on Friday, March 13, 2015.

Thank you for nominating a student for their contribution on campus. As you complete this nomination form, please follow the three-step process below. We encourage all nominators to be mindful of the following:

  • your statement should speak directly to the criteria for each award. Details on the criteria are linked here.
  • you can nominate a student for more than one category, however, your candidate will be considered solely for the award in which they are nominated
  • that the depth of information your provide will assist the commitee's consideration of the nominee. Provide us with specific examples about the nominee. Due to time constraints, the Award Selection Committee will not conduct additional inquiries regarding any nominee.
